什麼是種牙及植牙

在香港，「種牙」與「植牙」通常是指同一種牙科治療，即在牙槽骨中植入人工牙根，再在其上安裝人造牙冠，以取代缺失的牙齒。這種療程的主要目的，是恢復病人的咀嚼功能、美觀及牙齒排列，並且提供比活動假牙或牙橋更穩定、耐用的效果。植牙過程一般分為三步：先將人工牙根植入骨骼，待骨骼與牙根融合後，再安裝基台，最後固定牙冠。由於植牙需要良好的骨質支持與牙周健康，病人術前需接受詳細檢查與評估；術後則必須保持良好口腔衛生並定期回診，以確保植牙長期穩定。

甚麼情況下要植牙及種牙

  • Severely damaged teethWhen one or more teeth are missing, the teeth can no longer withstand normal biting forces, affecting chewing function or appearance, or even causing severe decay, cracking, or injury.
  • Long-term periodontitisLong-term periodontal disease can lead to loose or missing teeth, loss of chewing ability, weakened teeth, and even the spread of infection to other parts of the body.
  • Dental bridges or dentures are not suitableDentures can be uncomfortable and don't look or feel like natural teeth. Dental bridges, on the other hand, require grinding down adjacent healthy teeth, and cleaning underneath can be more difficult.
  • Tooth cracks and traumaAccidents, sports injuries, or falls can all cause teeth to crack. If the crack extends deep into the root, it cannot be restored by repair or root canal treatment.

Dental implants involve surgically inserting titanium metal into the alveolar bone to support the prosthetic tooth, and then attaching an artificial crown to the implant. They closely resemble natural teeth in appearance and function, replacing damaged or missing teeth and restoring chewing ability.

The dentist will first open the gum line and then use surgical instruments to implant the titanium alloy implant into the alveolar bone in a suitable location, providing a stable foundation for the crown that will be placed later. The implant surgery generally takes about 60-90 minutes to complete.

Immediate peeling and planting process

  • 初步評估Before performing an immediate implant procedure, the dentist will conduct a comprehensive oral examination, including taking X-rays and CT scans, to assess the quality and structure of the alveolar bone.
  • 拔牙和植入植體After determining that immediate implantation is suitable, the dentist extracts the tooth and immediately implants the implant into the socket left in the gum.
  • Suturing and RestorationAfter the implant is placed, the dentist will suture it and provide post-operative care instructions. Patients need to follow specific oral care procedures to promote recovery.

Do you always need dental implants if you lose a tooth?

Losing teeth doesn't necessarily require dental implants, but long-term tooth loss can affect oral health and overall health, often leading to chewing difficulties, unclear speech, tooth displacement, changes in facial structure, and alveolar bone loss. Dentists usually recommend timely repair to avoid these problems.

How long does the dental implant procedure take?

Dental implant surgery typically takes 60-90 minutes. If bone grafting is not required during the procedure, it usually takes only 4-6 months to complete. However, if local bone grafting is necessary due to insufficient bone volume, the procedure may take longer. If you have periodontal disease, it will need to be treated before implant surgery. The duration of periodontal disease treatment depends on the severity of the patient's condition.

What is computer-guided dental implantation?

Computer-guided dental implants utilize computer-aided design and manufacturing systems. Through 3D scanning, dentists can understand the patient's oral condition and the precise location and depth of the implant before surgery. Computer-guided implants provide each patient with a custom-made surgical navigator, guiding the dentist to place the customized, permanent all-ceramic crown or bridge implant into the patient's jawbone. By combining computer navigation with the dentist's clinical experience, the aim is to improve the accuracy of implant placement.

What are the advantages of immediate planting after peeling?

Immediate implant placement significantly shortens the overall treatment time compared to traditional implants. Patients no longer need to wait months after tooth extraction to have implants done, as it can be completed in a single surgery, reducing the number of visits. Due to the simplified surgical procedure, postoperative discomfort is also relatively reduced, and recovery time is shorter. In addition, this technique allows for immediate implant placement after tooth extraction, which helps stabilize the alveolar bone, effectively reducing the risk of bone loss and achieving the effect of protecting bone.

How long can implanted teeth last?

Many people believe that once they get dental implants, they can rest easy. However, dental implants are actually more accurately described as a "semi-permanent" solution. Both implants and dentures require regular maintenance and checkups, just like natural teeth. Only through regular professional checkups, coupled with proper daily care and usage habits, can dental implants function stably and sustainably in the long term.

Does dental implantation hurt?

Since anesthesia is administered before dental implantation, the surgery itself is generally painless. The main pain comes from the anesthetic injection and in the days following the wear off. The pain typically peaks around 48 to 72 hours after the procedure, significantly decreases after about a week, and usually disappears within two weeks.

How to care for plants immediately after peeling and planting?

Post-operative care includes maintaining oral hygiene. When brushing your teeth, be careful to avoid directly touching the surgical area and use mouthwash provided by your dentist to assist with cleaning. In addition, patients may need to take antibiotics to prevent infection, as advised by their dentist. Regular follow-up appointments are also very important, as they help the dentist continuously monitor the healing progress of the implant and ensure the overall treatment proceeds smoothly.

感染與發炎：手術過程需在牙齦與齒槽骨植入人工牙根，若清潔不當，可能導致牙齦紅腫、疼痛，甚至演變成植體周圍炎。

神經或鼻竇損傷：下顎植牙靠近神經管，上顎植牙接近鼻竇，若評估不精準，可能造成麻木、刺痛或鼻竇不適。

植體鬆動或斷裂：若植體未與骨頭牢固結合，或術後咬力過大、清潔不佳，可能導致鬆動甚至斷裂。
牙冠問題：植體上方的牙冠可能出現鬆脫或咬合不適，需要調整或更換。

牙齦退縮與美觀影響：部分患者術後牙齦可能退縮，牙齒看起來變長。

慢性疾病影響：糖尿病、骨質疏鬆等疾病會降低骨頭癒合能力，增加植牙失敗風險。
術後短期反應：腫脹、瘀青或疼痛屬正常修復過程，通常 1–2 週內消退
