「杜牙根」就是我們常說的根管治療，它是一種牙髓微創手術，目的是在牙齒因深層蛀牙、外傷、齒裂或嚴重牙周病導致牙髓（內含神經與血管）受到細菌感染或壞死時，透過清除牙髓腔內的細菌並消毒填充，阻止感染擴散到牙周組織和牙槽骨，避免這些組織受破壞，從而保留自然牙齒並維持其功能。換句話說，杜牙根就是「救牙」的最後防線，讓原本可能需要拔除的牙齒能繼續使用。

Severe tooth decay that has penetrated to the pulp
When bacteria have invaded the dental pulp, causing infection or necrosis, and fillings can no longer repair the nerve, the tooth root must be sealed.
Irreversible pulpitis
When experiencing prolonged, severe pain (such as waking up at night due to pain, or pain when chewing), or when swollen gums or pus discharge are observed.
Pulp necrosis (even if painless)
When the nerve of a tooth is dead but there is still a chronic infection, bacteria and pus accumulate at the root of the tooth.
Multiple failed fillings or persistent toothache
If multiple fillings are ineffective and damage to the dental pulp is observed, root canal treatment is necessary.

Root canal treatment, also known as tooth extraction, is a treatment method used to preserve teeth when the dental pulp (dental nerve) is inflamed, necrotic, or infected. It involves removing the damaged pulp, disinfecting the root canal, and then filling and sealing it to prevent bacterial re-entry, making it an important means of avoiding tooth extraction.

In most cases, braces are recommended because teeth become more fragile, and braces can protect teeth and prevent them from breaking.
Generally, one to two treatments are needed. If the bacteria are not completely eliminated during the first treatment, a second root canal treatment may be required. This involves removing the old crown, core, etc., and cleaning the root canal again. Complex cases may require more treatments.
The time required for root canal treatment depends on the severity of the infection and the complexity of the tooth's structure. If the infection is minor and the root canal structure is simple, an experienced dentist can complete the entire procedure in one session, taking approximately 60 to 90 minutes. Conversely, if the tooth infection is severe or the root canals are numerous and curved, it may require 2 to 3 sessions, each lasting approximately 30 to 60 minutes, to gradually remove bacteria and monitor the healing progress.
Since anesthesia is administered before dental implantation, the surgery itself is generally painless. The main pain comes from the anesthetic injection and in the days following the wear off. The pain typically peaks around 48 to 72 hours after the procedure, significantly decreases after about a week, and usually disappears within two weeks.

Pulp capping is a method of protecting the vital dental pulp. It involves applying a preparation with restorative effects to the pulp, covering the dentin near the pulp or the exposed pulp wound, to protect the pulp and allow the lesion to heal. Pulp capping can be divided into direct and indirect methods. Indirect pulp capping is suitable for reversible pulpitis caused by deep caries and does not expose the pulp. Direct pulp capping exposes the pulp tissue.
Pulp capping is a more invasive procedure than root canal treatment, and it is also less expensive. However, if the patient does not maintain good oral hygiene habits after the procedure, root canal treatment may still be necessary in the future.
If traditional root canal treatment fails to completely remove the infected and necrotic pulp tissue, the patient may need to undergo periapical surgery, which is a last resort to save the tooth. The dentist will perform the surgery under a microscope, meticulously removing the infected and necrotic root tip and placing a root canal filling to prevent bacteria from entering and thus preserve the tooth. Before undergoing periapical surgery, the patient should be examined and diagnosed by a dentist to assess whether surgical treatment is necessary.

單次完成：如果感染不嚴重、牙根結構簡單，醫生可能一次就能完成清除、消毒和填充。
2–3 次：較常見的情況。第一次清除牙髓和細菌，第二次消毒並觀察癒合，最後一次封閉和修復（如加牙套）。
更多次數：若牙齒感染範圍大、牙根管複雜或需要觀察癒合，可能需要 4 次或以上。
